We carried out an announced comprehensive inspection at The Hextable Surgery on 25 July 2017. The overall rating for the practice was requires improvement. This inspection was an announced focused inspection carried out on 10 November 2017 to confirm that the practice had carried out their plan to meet the legal requirements in relation to the breaches in regulations that we identified at our previous inspection on 25 July 2017. This report covers our findings in relation to those requirements and also additional improvements made since our last inspection.

Overall the practice is now rated as good.

Our key findings were as follows:

  • The practice was able to demonstrate that they followed national guidance on infection prevention and control.
  • Risks to patients, staff and visitors were assessed and managed in an effective and timely manner.
  • The practice was able to demonstrate they had adequate arrangements to respond to emergencies.
  • The practice had an action plan to address data from the Quality and Outcomes Framework (QOF) where the results showed the practice management of patients with long-term conditions were mixed.
  • The practice was able to demonstrate that staff were up to date with relevant training and that this was on-going.
  • The practice was able to demonstrate they had a clear system to follow up on patients who were referred to other services.
  • Governance arrangements were effectively implemented.
  • Medicines management procedures had been reviewed to ensure an effective process for managing medicine alerts from the Medicines and Healthcare products Regulatory Agency (MHRA) and other notifiable incidents. There was a system to ensure the timely review and action of safety alerts.
  • The practice was able to demonstrate that they kept records to show how they learned from complaints or implemented changes as a result of complaints.
  • The practice had identified 47 patients on the practice list who were carers which is 1.3% of the practice list. This is a significant increase since the last inspection.
